Home Care Today
SPRING/SUMMER 1982
Today, the health statistics tell
us that there are 24 million people
over 65. Within 50 years that figure
will double. Those same statistics
tell us that the over 65 have an
average of 37 sick days a year and
65 to 75 percent of these sick days
are the result of chronic rather
than acute problems.
Although the VNA cares for patients of all ages, 66 percent of our
patients are over 65 years of age.
Their illnesses range from heart
disease to arthritis; many have sev-:eral conditions: it is not unusual to
have a patient with an admitting
diagnosis of heart disease complicated by diabetes and high blood
pressure. Mobility may be limited
because of arthritis. Impaired vision
and/or hearing may further complicate the patient's life.
This is where the VNA provides
a continuum of care. Perhaps there
has been an acute episode for
which the patient has been hospitalized. Discharge planners and
continuing care coordinators insure that when the patient is ready
for discharge the VNA and other
community agencies will provide

VNA nurses, in conjunction with
the patient's own personal physician, will assess and evaluate the
condition and coordinate the necessary services: skilled nurse, home
health aide, homemaker, speech
therapist, physical therapist, occupational therapist and medical social worker are all readily available.
What's equally important is the
VNA's role in eliminating future
problems. Nutrition is vitally important to the elderly and often they
don't eat properly. Perhaps there is
the isolation that comes with difficulty in moving about and getting
out of a home to visit friends. Per-
haps there is depression which
can be helped with medications.
The visiting nurse sees this patient
on a regular.basis, confers with the
physician and the other health professionals and can intervene to
help this patient back to a more
independent status before th e
downward spiral begins.

people live longer, our job becomes

On Outpatient
Oxygen Therapy

Now here is the cha Ilenge of the '80s
more evident than in the field of home
health care.
People want to stay in their own
homes as long as they can, and studies have shown that it is more econorn ic to maintain a person in the
home. In this area of the county we are
also faced with a continued shortage
of nursing home beds. Home health
agencies are devising ways of reaching people who need our assistance
and who are appropriate for admission to the caseload.
There are more than 500 VNAs in
the United States, of which the Stamford agency is one, certified by Medicare for home health care. Last year
almost one billion dollars was spent
by Medicare for reimbursement of
patient services. Indeed, 72 percent of
the professional visits and 55 percent
of the home health aide visits are
reimbursed by Medicare at the Stamford VNA.
Our challenge remains to provide
care for the most number of people
within the budgetary constraints put
forth by Washington. The number of
people needing home care will increase. The services offered will become more complex. There will be
greater emphasis on appropriate utilization of these services and at the
same time insuring that patients receive every assistance to remain in
their homes and avoid i nstitutional ization.
There is a unified voice within home
care agencies in order to help in drafting legislation and amendments to the
Medicare laws and regulations. What
the regulators do in Washington and
in Hartford will be influenced by the
information they receive from agencies such as the Stamford VNA. Only
then can we assure that our patients
will be cared for and the challenge of
the '80s will be met.
EDITOR'S NOTE: Some of the
patients seen by the Visiting Nurse
Association have a diagnosis of
chronic obstructive pulmonary disease from emphysema, asthma or
a similar respiratory problem.
Sometimes these patients are on
long term use of oxygen in the
home. Here is the latest information on this treatment.
"The long term use of oxygen
was first described in 1938. However, it has only been in recent
years that home oxygen therapy
was made practical by space-age
technology.
"The treatment of chronic respiratory insufficiency by means of
low-flow oxygen has recently been
reported from two large multicenter collaborative trials from
England and the United States.
The British study included 87 patients; oxygen was given for 15
hours daily including sleep to the
study group; the control patients
received the same medications but
no oxygen at all. The oxygentreated group had considerably
improved five year survival compared with the control group, although no difference was noted
until 500 days had elapsed.
"The American study included a
similar population. Continuous {essentially 24 hours) oxygen was

. benefits in addition to longersurvival: improved quality of life and
reduced need for hospitalization.
"Oxygen therapy is safe. Surprisingly there has not been any
oxygen related accident or fire
reported in patients on home oxygen. {Lloyds of London charges
no increase in rates for household
insurance if oxygen is used at
home.)
"Oxygen therapy is an expensive modality of treatment and
careful patient selection becomes
a financial reality. In Stamford,
continuous nasal oxygen at two
liters per minute costs close to
$500 per month for a reservoir and
portable walker. These figures have
more impact when one realizes
there are approximatety eight to
nine million COPD patients in the
United States alone. Almost
500,000ofthese patients are receiving disability benefits.
"Further studies are being
·planned to evaluate the minimum
number of hours of oxygen therapy that will still give substantial
benefit. For the present, however,
we can offer selected patients with
advanced COPD a safe and effectivetherapy modality which results
in a better quality of life and may
increase life span as well."

Then, as now, the visiting nurse

Aide employed at the agency. Yvette Carp, RN,

Isabella Hines, HHA, 5 years; Pamela Smith, HHA,
5 years.
Also honored was Helen Beaudoin who volunteers in the Home Health Aide Office.
Harriet Shaub, RN, received special recognition. She entered the doors of the VNA at its first
office on Worth Street in 1928, and has been with
the agency since then with time off to raise a
family. Now retired, she still lends her talents and
expertise to the agency on a regular basis.

The VN A nursing
staff in 1937
A half century ago, in 1912, the VNA first opened its doors. With a staff of one nurse, its
headquarters were three small rooms in the Richmond House on Worth St. By 1915, a dental clinic
had been opened, and the• nursing staff had grown to three. Tuberculosis work began in 1916.
Nursing visits were made on foot, by trolley, or by bicycle, until 1917 ... when a car was
presented to the VNA as a gift. The present VNA building at 60 Guernsey St., dedicated in 1931,
was paid for by gifts from friends in the comm1mity.
Tbe VNA was formed by Stamford people and hc1s continued its work through the support
of Stamford people. Their generosity, through th1~ years, has been echoed in the devotion of the
Visiting Nurse to her patients ... and to the hea ~ th needs of the city. For your gifts ... your
concern for others ... the Visiting Nurse and those she serves are .deeply grateful.

first annual Seal S~le of Stamford's own Christmas Seals. Raised in the
drive, entire proceeds of which will be used to pr~vicje nursing care (Ind

REPORT
ON
AN AVERAGE of .64 times per day
every working day of 1954 in Stamford someone in need of help received
it from one of the nurses of the Visiting Nurse Association. More than
17,700 visits were made, serving over
1,500 families in the community.
It is hard to encompass in a few
words the wide scope of the VNA's
work and the statistics which follow
cannot tell its full human worth. For
who can estimate the value of skilled
nursing care to the infant with an
acute illness, a · child with rheumatic
fever, a crippled boy or girl, a teenager in the long days of convalescence
after major surgery, or the old person,
perhaps bed-ridden, with heart disease, arthritis or eancer. Yet it is to
people like these of all ages, many of
whom could tum nowhere else for the

But bedside nursing care is not the
whole story of VNA. In its headquarters are held tuberculosis diagnostic
clinics, x-ray and ~ducational programs in conjunction with medical
specialists from the Connectfout Tuberculosis Commission, for the better

�control of TB in Stamford where 201 diagnosed
cases were carried by the VNA and 922 patients
were served in the VNA clinics. Lectures and
instruction classes are given for new and expectant parents and, in 1954, 433 young mothers
were guided in the healthful care of their babies.
A health service is provided for infants and preschool children. Dental and orthodontia clinics
for children are also conducted along with a
contin~ing program of health education in all
its phases.
In less than a third of the visits made by the
VNA, the patient could · pay the full fee, in
another 13 Yi % of the visits a partial fee was
paid. To carry out the remainder of its work so
vitally important to the well-being of the community, and yet maintain its free, voluntary
character, independent of governmental or other
outside fiscal control, the VNA must rely largely
on the generosity of its individual members
whose understanding and support have enabled
it to serve for 42 years in the best American
tradition.

Christmas Seal Sale
The VNA is the local agency of the National
Tuberculosis Association; and the seal sale is
conducted bv the VNA's Tuberculosis Christmas
Seal Committee.
During the 1954 seal sale, 1,795,000 seals were
bought by the people of S.t amford. Out of each
dollar given through seal sales, 84 cents remains
here in Stamford for the work of tuberculosis
control.

Direct personal service is the heart of VNA
activity, and the effectiveness of this service depends largely upon the visiting nurse herseH. Her

meet the highest standards of her profession but

marked the 40th year that the VNA
has brought its health services to the people of
Stamford. Because only a small portion of those
who have benefitted from the VNA services are
able to pay in full, almost half of the visits were
provided free. This important contribution to the
welfare , of the community has been made possible through the donations of members and the
generous cooperation of the medical and dental
professions. The financial aid of the YNA membership and the counsel and guidance of the
professional advisors, are a source of real inspiration to the working staff; continued support
is essential, of course, to permit the YNA to carry
on its vital work.

NURSING SERVICE
DESPITE war's toll of personnel, which in 1945 brought a
new low in the number of nurses on its staff, the Visiting
Nurse Association strove to serve Stamford with adequate
nursing care. Almost twenty thousand visits to patients in their
homes, bringing the record since the founding of the association in 1913 to well over half a million visits, attest the success
of their efforts.
Following the instructions of physicians, the staff nurses
attended adults and children, the acutely ill, and

t~e chronically sick, those suffering from communi- _ '\
cable and non-communicable diseases; they gave
bedside care, administered injections, changed •
dressings, and carried out various other nursing
treatments. They carried on educational work at
headquarters and in the homes, furthering a program of general family health supervision.

1,750 families of Stamford.

DENTAL CLINIC SERVICE
(HILDREN under sixteen years of age, whose families
could not afford the services of a private dentist, were
served by the dental clinic, given treatment of vital importance
to general health and taught proper care of the teeth. Two
modern, well equipped offices are provided for this clinic.
A staff of fifteen dentists, several recently returned from the
armed services, gave generously of their time and skill, assisted
by the staff hygienist.

MATERNITY SERVICE
KNOWLEDGE and ·skill combined to give Stamford better
and healthier mothers and children.
Expert instruction was given to the expectant mothers who
attended the classes in prenatal and infant care, given in series
of eight weekly lectures throughout the year. They learned
how to care for themselves and their babies; they benefitted
from class discussions of particular problems.
Skilled supervision was given to expectant mothers in their
homes, and nursing care and supervision to
mothers and their newborn infants.

TUBERCULOSIS SERVICETHIS service is made possible by the annual sale of Christmas
Seals.

Under the leadership of the teachers, the major portion of
the sale is carried on by the school children of Stamford. The
proceeds of the campaign made it possible to meet the entire
cost of the program.
Special features of this service are early diagnosis; extensive
health education, follow-up treatment and recommendations;
as well as the discovery of contacts and arranging for necessary examination and care. This entails not only clinical supervision, but carefully planned home visits.
